Under the new rules, students are required to submit the following information and certificates to the dean's office:
1) Information from the Register of Sexual Offenders:
- from the Register with limited access (information in PDF format),
- from the Register of the State Commission (information in PDF format).
The Register of Sexual Offenders is maintained in an IT system, information can be obtained online at: https://rps.ms.gov.pl/en-US/Public#/
Please send certificates by email in PDF form.
2) Certificates from the National Criminal Register (KRK)
A certificate from the KRK can be obtained
- directly from the National Criminal Register Information Bureau located in Warsaw at 100 Czerniakowska Street or the KRK information point (the list of information points is available at: https://www.gov.pl/web/krajowy-rejestr-karny/materialy). We recommend this method; it is faster as the certificate is usually issued the same day and you don't need to have qualified electronic signature.
- or online after registering on the MS Krajowy Rejestr Karny e-Platform at: https://ekrk.ms.gov.pl/ep-web?lang=en. Please note that in order to apply electronically for the certificate, one needs a qualified electronic signature, a trusted signature (podpis zaufany) or an electronic personal signature (podpis osobisty).
- For certificates obtained in paper form, please deliver the documents directly to the Dean's Office.
- If the certificate was obtained online after registering on the MS National Criminal Register e-platform (https://ekrk.ms.gov.pl/ep-web), please send it from your student account to the Dean's Office. By obtaining the certificate online, you will receive a zip file, which will contain 3 files (PDF visualization, XML, electronic signature). All 3 files should be sent in an email.
The certificate cannot be older than April 15, 2024.
3) In the case of a foreign student, he/she shall also submit information from the criminal register of the country of citizenship obtained for the purposes of professional or volunteer activities related to contacts with children.
According to the attached Announcement, the deadline for delivery of documents is:
- 31.10.2024 - Information from the Register of Sexual Offenders and a certificate from the National Criminal Register and a statement about the country or countries of residence during the last 20 years.
- 29.11.2024 - Information and certificates from foreign registries.
Failure to provide the above information, certificates and declarations to the dean's office within the above deadlines will result in the student not being admitted to classes/internships due to the student's fault.
